The northern United States (Union) fought the southern (Confederacy). The Union army lost about 360,000, the south about 260,000, the rest (about 50,000) were civilians.

hm... that's an interesting question. The Influenza epidemic was later, 1918. I hadn't heard Influenza affected the Civil War too. It certainly affected WWI, though.
